Did you guys hear the transport minister(news at 1) for transport release the KQ west africa crash report attributing the crash to pilot error..?

Makes for interesting reading and yes the pilot is blamed. It seems the faults were:
-departing Douala in a bit of a hurry, in darkness without clearance
-behaved as if autopilot engaged even though it failed to activate
-despite poor external visuals, pilots failed to recognise, from the instrument panel, aircraft's increasing bank from 11° to 34° to the right
-surprised then panicked when alarm rang becoming disoriented thus aggravating the situation

Sad situation though...only 1min 42secs later.

This accident boils down to crew cooperation and especially how senior aircraft captains relate with their junior first officers and what role the company (operator)ensures that it is done. It is not surprising nor strange to find the age difference between between crew to be as much as 40 years. But you must understand that this is not necessary bad. A captain who is 64 years is very experienced and it is necessary that he passes on his skills to younger officers who would be 25 years or so.The problem is that this captain may think he /she is a Mr/Ms 'Know It All' and a young pilot,fresh from flight school knows little to point out a mistake by a captain. A number of accidents and especially if you've been watching air disasters on DSTV, have been pointed out by the copilots but only for the captain to say, 'I know or I can see'. So what is the remedy for this? A number of airlines are coming up with a new approach for the crew to call each other by first names and for copilot to take over control if a mistake is pointed out three times without action. Of course there is the problem of insubordination and this where the airline comes in to protect the young pilot.
