NHIF forced health plan a bad idea

I think under this new health plan we will see more deaths and poor medical practice than ever before. I wonder if hospitals will survive after the multibillion shillings malpractice laws suits that will follow. Here are a few extracts from today's Business daily;

Pressure to make profits by the hospitals that partner with the NHIF under the new outpatient scheme could force them to offer sub-standard services.

The hospital is required to manage the funds while keeping the surplus as profits; institutions that exhaust the payments will thus make a loss.

This, the experts say, will be open to abuse in weak audit, pointing out that there will be temptation to spend less with eyes on profits, leading to poor services.

“The problem [with this financing model] is that the only way the hospital can make money is to hope that no one covered falls sick and if they do, they don’t spend much. So they tend to give very poor services to save costs,” said Mr Peter Nduati, the CEO of Resolution Health East Africa.

I honestly have no real problems with this plan, but would have proposed that:
1. Make the deductions tax deductible
2. Make it a percentage of ones pay - I shouldn't pay the same as the CEO of a major bank

And should you get the same kind of treatment with him? Same hospitals?
Never count on making a good sale. Have the purchase price be so attractive that even a mediocre sale gives good returns.

GG et al,

The major problem with this is the absolute lack of details which I can assure you is not by mistake. I have just learned that NHIF is proposing to pay hospitals and clinics a capitation of Ksh 2,000 per year for a family of five. They will then expect the clinic to provide what they are calling 'comprehensive healthcare' for a whole year for Kshs 400 per member. Even the Maasai's selling herbs in the streets will struggle to provide their concoctions to anyone for this amount per year. Granted that the capitation model will blunt the effect a little, I doubt any hospital or clinic will balance their books for this amount. If any hospital accepts it, I can assure you that the quality of services will be commensurate with the amount NHIF is paying. This is the reason the CEO of Resolution talks of making money by hoping no one falls ill!!

NHIF therefore MUST increase the capitation payout to clinics, something I doubt will happen given the 'i lunch at serena' attitude of the minister. I am expecting the govt to bully all public hospitals to accept the scheme with no takers (or at least very few) among private hospitals and clinics.

So GG and wazuans/friends, welcome to Mathari hospital for your dialysis or any other procedure sir. The doctor and nurses are out for lunch until 3pm so please be patient. You can lie on the floor and you will feel much better. The machines have been out of order for a while but we are in the middle of the procurement process for new ones. Nkt
